AP made a bad one about a decade ago.

They sent a running White Sox gamer and then sent about 10 corrections within the next 30 seconds.

The first story said "Frank Thomas likes to take pitchers deep into the count."

The problem was, they forgot the O in "count"

It was actually an excellent mid-major that spent quite a bit more on its editing operation than most papers its size and every page with news content had been proofed, including features sections days earlier. I've never seen a paper where the masthead wasn't a graphic instead of something you had to type in. How damn cheap can you be?

I think most papers actually call the place where the Valley Newss thing appeared the flag. The masthead is where the list of editorial staff goes.

When there are certain things that almost never or actually never change over the course of a year, I think a lot of people don't train ourselves to look. I'm sure a number of us normally just assume something's right or that someone behind us in the pecking order will notice and MAKE it right.

That said, an egregious enough mistake will probably cause a lot of people to start checking everything for a while.
