Sal posted another YT video yesterday that provides more details with two key points:
1) the ship has a large turning radius so it needs to back up further so that when it begins moving ahead, it can clear the berth it just left, and
2) the ship has a Continuous Pitch Propeller (CPP) which, while the propeller shaft continues to spin only in one direction, allows the propeller blade pitch to change so that rotation is converted between forward or reverse thrust.
He also has talked about the wind and current at the time pushing the ship toward the bridge. He’s also described the tug operation a bit.
So, some possibilities are mechanical/electrical failure of the CPP or human error in not engaging the CPP to forward thrust.
