In the "scoring plays" section of your box score, i.e. 1:37 — Smith 2-yard run. (Jones kick.)

How do you write up a hook-and-ladder play?
 
Not sure if this is right but this is what the ESPN.com box score has on the play from Boise State-OU a couple years ago:

7 JERARD RABB 35 YD PASS FROM JARED ZABRANSKY (ANTHONY MONTGOMERY KICK)

The play-by-play described it like this (The play started at the 50):

Jared Zabransky pass complete to Drisan James for 15 yards, lateral to Jerard Rabb for 35 yards for a TOUCHDOWN.

Edit: This appears to be right. The box score from the Utah-Pittsburgh Fiesta Bowl in 2005 where there was a similar play also records only the second part of the play.
 
I had two of those last night. None for a TD, but I had to explain it when I was going over the end-of-game stats, which is why a guy had 1 22-yard catch, but 1 catch for 39 receiving yards.
 
The way I was told many years ago to do it in the scoring summary:

Jones 15 run with lateral after Smith 25 pass from Brown (Johnson kick)

The explanation was each guy gets credit for the yards he gained while the total distance of the play can be easily figured by a reader with fourth-grade math skills
