There was a fatality in Delaware:

Zip line fall leaves Delaware woman dead - CNN.com

My boys and I just went through a zipline near us a month ago that looked very similar. It was a blast, we were very high up but every step of the way I made sure we were all "clipped" in and the two instructors with us seemed to be very careful as well.

Hoping that there was "pilot" error (sad for woman) rather than institutional error.

Anyone else done a similar experience?

Terrible story. Seems like more and more zip lines are popping up.
One just opened at Niagara Falls- not for the faint of heart
