Don't use Word for anything that requires attractive graphics?

Understood, but I'm talking about a mailing label.
I need a label template for an Avery 5136 that includes my logo. Then I and others can use the tempalte for mailing labels. It has to be in Word because that's the most readily available to software for everyone else. Only a few of us have CS4.
So the template has to have the logo in it and a text area where people can type in the address.
I've tried a bunch of different things with Word, and I can get it almost correct. It just goes askew with the alignment on the labels.
I've done it by making it a fillable form with Adobe LiveCycle Designer, but the program doesn't like it. I got a prototype dopne, but it wasn't quite right. When I tried to do it, LiveCycle wouldn't let me add the text box.

It's all quite vexing.
