I have a sheet I made on Quark for 11x14 paper. It's become that big because of high schools and jucos that started thinking they need to pass 35 times a game. In fact, I've still run out of places to put passing in a couple of juco games because even at that size, I could only put 63 passes on it.

I've been using this one for a couple years. For the most part, it's great, but I have a couple gripes with it. For one, there's no real easy way to keep track of QB stats. On the legal size, the passing running total is too small, so I'll keep it on a separate sheet of paper, along with penalties/yards and first downs. Not too big a deal, but if it had passing totals laid out the same way the rushing and receiving ones are, it'd be perfect.

I'd have to dig it out of a box first, which shouldn't be too difficult.

Hard copies only. And since it's 11x14, it won't fit on my scanner.

I've made up an Excel spreadsheet that I use for game stats ... you can either print it out on an 8x14 sheet of paper, or run it on your laptop for live updates as the game goes along ... there are some on SportsJournalists.com who have used it, and I've gotten no complaints (yet) ... PM me with your e-mail address, and I'll send you a copy ...
