Was looking at the locked thread, threw "soccer scorebook page" into Google and found this PDF.

When I was SIDing, we played a few games at a field without power, thus no computer to keep stats (the game lasted longer than my battery). Here's the form I used to keep stats during those games; it's pretty detailed. The second page of the PDF is a game protest document; you probably won't need that.
